So it's such a huge difference. I want to talk about 73 00:06:04.959 --> 00:06:12.199 an article that just appeared by two authors, Kimberly Kopecki and Toby Campbell, 74 00:06:12.360 --> 00:06:16.839 in a journal called The Oncologists. This is a medical journal published by Oxford 75 00:06:17.000 --> 00:06:24.800 and just appeared from the Department of Surgery and Division of Hematology Oncology in two 76 00:06:24.920 --> 00:06:28.360 famous hospitals in America. And they start out the article, I read to 77 00:06:28.360 --> 00:06:35.040 you a staggering sixty nine percent of patients receiving non curative chemotherapy for metastatic lung 78 00:06:35.079 --> 00:06:43.920 cancer and fifty percent of people receiving chemotherapy for advanced gastroesophageo cancers, liver cancers, 79 00:06:43.959 --> 00:06:48.519 billary cancers, or pancrease cancers report that the goal of therapy was to 80 00:06:48.639 --> 00:06:55.879 cure the cancer. Eighty one percent of patients with metastatic colorectal cancer believed that 81 00:06:55.959 --> 00:07:01.439 chemotherapy was to cure the cancer. And they say, why does this disconnect 82 00:07:01.519 --> 00:07:05.240 exist and what might we do about it? Why? Because the goal is 83 00:07:05.319 --> 00:07:10.720 not to cure the patient. But patients are being confused by their doctors. 84 00:07:10.720 --> 00:07:15.240 When doctors say, oh, it's powative therapy. Most patients don't understand that 85 00:07:15.360 --> 00:07:20.759 word means non curative. Why aren't the doctors cancer doctors being clear and saying, 86 00:07:21.000 --> 00:07:25.839 look, you've got a cancer that cannot be cured with chemotherapy. If 87 00:07:25.839 --> 00:07:29.279 you want chemotherapy, that might slow it down for a few weeks or a 88 00:07:29.279 --> 00:07:32.279 few months before it grows back. That's why you might want to take chemotherapy. 89 00:07:32.600 --> 00:07:38.000 But understand, with chemotherapy, most chemotherapies have side effects because it's kind 90 00:07:38.000 --> 00:07:41.600 of putting up poison in your body, like putting your body in a bathtub 91 00:07:41.600 --> 00:07:45.079 of poison and expecting nothing bad to happen. I can tell you, for 92 00:07:45.199 --> 00:07:49.199 most people, something bad does happen. And as his authors point out, 93 00:07:49.240 --> 00:07:57.079 why do doctors not tell patients directly what's going on? When doctors and they 94 00:07:57.079 --> 00:08:01.800 say this quote the article, the word pallative is routinely used in discussion with 95 00:08:01.879 --> 00:08:07.120 patients and their families, despite the fact that the majority of patients are unfamiliar 96 00:08:07.120 --> 00:08:11.560 with what pawative means. Do you know what powative means? If you're getting 97 00:08:11.560 --> 00:08:13.519 treatment, oh, we're gonna give you a powative therapy, it means we're 98 00:08:13.560 --> 00:08:16.160 not gonna cure you. We're not even gonna try to cure you. It's 99 00:08:16.199 --> 00:08:20.360 not possible. That's what they mean. But the doctor, rather than saying, 100 00:08:20.399 --> 00:08:22.959 hey, missus Jones, We're gonna give you a powat of therapy. 101 00:08:22.000 --> 00:08:26.120 They make it sound so fantastic. I've had patients come in and say, 102 00:08:26.120 --> 00:08:30.399 doctor Liederman, can you give me palwative therapy? Powat of therapy means non 103 00:08:30.519 --> 00:08:35.600 curative therapy. It's exactly what most people don't want. Most people want curative 104 00:08:35.639 --> 00:08:37.600 therapy if they can have it. And the authors go on to say, 105 00:08:37.799 --> 00:08:46.200 when ambiguous terminology is used, the need to communicate a bad prognosis can be 106 00:08:46.200 --> 00:08:50.279 avoided. So maybe, and I think it's true that doctors don't really want 107 00:08:50.320 --> 00:08:54.120 to tell patients directly what the truth is that the treatment will not cure the 108 00:08:54.159 --> 00:08:58.159 cancer. Why don't they have the strength and the stamina and the backbone to 109 00:08:58.279 --> 00:09:03.879 tell the patients the truth? What do you use these words that most patients 110 00:09:05.120 --> 00:09:11.879 just don't understand? And the authors go on to say, for patients, 111 00:09:11.080 --> 00:09:18.000 the use of unclear language can propagate uncertainty regarding the indications for treatment and allow 112 00:09:18.120 --> 00:09:24.000 them to imagine that what they're getting desperately hoping for cure is actually a shared 113 00:09:24.080 --> 00:09:28.639 goal when it isn't. The patients might hope for cure, but the doctors 114 00:09:28.679 --> 00:09:35.159 aren't even offering that, and they're obscuring that with bad language that most patients 115 00:09:35.240 --> 00:09:39.279 don't understand. And the authors go on to say, we recommend a single 116 00:09:39.360 --> 00:09:43.440 word substitute to call it what it is, to say, this is non 117 00:09:43.639 --> 00:09:48.000 curative therapy instead of pawative therapy. Most people don't know what powative is, 118 00:09:48.240 --> 00:09:50.480 but most people do know what non cure means. We're not going to cure 119 00:09:50.519 --> 00:09:56.000 you. Why don't the chemotherapy doctors and some surgeons and others use that word. 120 00:09:56.039 --> 00:10:01.240 Why are they being Why are they fudging their life language to obscure the 121 00:10:01.320 --> 00:10:03.080 patients? And the author is going to say, why does it matter? 122 00:10:03.480 --> 00:10:07.120 Before a patient can make decisions, set goals, or cope with the reality 123 00:10:07.120 --> 00:10:13.080 of a new cancer diagnosis, they need first to understand the recommended treatment plan 124 00:10:13.519 --> 00:10:16.720 and how to recognize how it is and how it's not expected to alter the 125 00:10:16.759 --> 00:10:22.039 course of the disease. Is it to cure the patient or not? Pawative 126 00:10:22.279 --> 00:10:26.159 means not to cure the patient. So why do people take it? And this is what's so important. The authors conclude, it's been 149 00:12:20.879 --> 00:12:28.399 suggested that structured communication can overcome the ambiguity of the term palliative chemotherapy.
